What is Subcision?
Subcision is a minor surgical technique without incisions that is carried out at Dadu Medical Centre by expert skin doctors. It is performed to treat depressed acne scars; distensible scars caused after surgery, chickenpox scars or injury; depressed skin grafts; wrinkles.
As an acne scar treatment, subcision alone can treat or the results can be improved by combining the technique with other procedures such as micro-needling, use of topical retinoids, TCA cross, Fractional laser technique.
What does Subcision entail?
In this procedure, a dermatologist administers a thin needle in the hypodermis of the skin just beneath the scar. Insertion of the needle leads to breaking and losing the fibrous bands that attach the scars to the underneath tissues. Thus the depressed scars get lifted from the skin and stimulate the new collagen production at the wounded site of needle insertion.
This treatment is suitable for rolling and boxcar acne scars in Delhi but may not be able to treat keloid scars and hypertrophic scars. Individuals who have medical conditions such as bacterial or viral infections, active acne, and blood clotting disorders or take oral retinoid are not recommended to undertake this procedure.

How can an individual know if Subcision is needed?
A person who has multiple boxcars (rounded craters in the skin with sharp vertical edges) or rolling scars (pits in the skin with sloped sides) that do not disappear upon skin stretching or get smoothened upon smiling require subcision. It is also often carried out in case of very little tethering to acne scars.
What to expect during the treatment?
At the initial consultation, the skin experts at Dadu Medical Centre medically reviews the clients, discusses with them their expectations, and explains the entire treatment plan to them. Before commencing with the main procedure, the scarred skin is cleaned properly by a dermatologist to remove the make-up and dirt. The margin of the scars is marked to well-define the area to be treated under the overhead lighting which helps delineate depressions. Lastly, the treatment site is numbed by applying topical local anesthesia about 30 minutes before starting the procedure to avoid pain or discomfort felt during and after the treatment.
The dermatologist inserts a tri-beveled needle (18 or 20 number gauge) or a Nokor needle (with a small sharp blade at the tip of a short fine needle) into the skin dermis horizontally (at an acute angle) just below the scar. It is gently moved parallel to the superficial skin just underneath the scar in a back and forth, fan-like motion to take out the complete scar which is connected to its underlying skin tissues. The needle is again moved in the same way after rotating it by 90 degrees. This is called fanning subcision. Lastly, the needle is taken out and circumferentially squeezed back around the point of exit of needle insertion so that a large hematoma is not formed because of bleeding.
In the case of small areas which require subcision, Cannula is used that is inserted through a tiny hole made by a needle. A scar can be taken out either by carrying subcision in one direction or by creating a crosshatch.
It is an outpatient procedure. Best results are visible in 1 to 3 months as the skin continues to remodel itself. The person generally requires multiple sessions with a spacing of at least 4 weeks. The number of repetitions needed depends upon the shape and size of the scars and the surgical instrument used.
The cost of acne scar removal with subcision in Delhi depends on the number of scar tissues removed and the number of sessions provided.
Immediately after the treatment, the wounded site after needle insertion is healed by applying ice or manual pressure for several minutes. This way the inflammation and bleeding can subside. It takes about 1 to 2 weeks for the pain, bruising, and swelling caused by needle insertion to heal. An antibiotic or anti-inflammatory is prescribed to help with the recovery. A person may apply makeup to camouflage the bruised area.
